Based on the criteria of selecting “articles most likely to change clinical practice,” one article of significant interest is included in this weekly update.

 

USPSTF Recommendation Statement on Prostate Cancer Screening


The United States Preventive Services Task Force (USPSTF) updated their 2002 recommendation on prostate cancer screening.  They concluded there is insufficient evidence to recommend for or against prostate cancer screening in men < 75 years old (I recommendation). Prostate cancer screening is NOT recommended in men ≥ 75 years or in men with life expectancy < 10 years because harms outweigh benefits (D recommendation). This represents a change from the 2002 conclusion of insufficient evidence for or against screening in this group. They also noted that, if PSA screening does reduce mortality (which has not been established), prostate specific antigen (PSA) screening every 4 years would be as beneficial as annual screening. They noted harms associated both with screening and treatment (including detecting patients who would never have developed symptomatic cancer) (Ann Intern Med 2008 Aug 5;149(3):185). DYNAMED EXTRA – ACR Treatment Guidelines for DMARDs for Rheumatoid Arthritis

 

The American College of Rheumatology (ACR) has published updated evidence-based guidelines for the use of disease-modifying antirheumatic drugs (DMARDs). The guidelines provide recommendations for rheumatoid arthritis patients who have never received DMARDs (stratified by disease duration and activity) and patients who have previously used DMARDs (Arthritis Rheum 2008 Jun 15;59(6):762 PDF).
