Last week 588 articles were evaluated via DynaMed’s Systematic Literature Surveillance and 201 were added to DynaMed content.

 

Based on the editors’ criteria of selecting “articles most likely to change clinical practice,” one article of significant interest was selected for the DynaMed Weekly Update.


Implantable Cardioverter-Defibrillators Do Not Appear to Reduce Mortality in Women with Heart Failure

 

Implantable cardioverter-defibrillators (ICDs) are frequently advised for patients with heart failure who are at increased risk for sudden cardiac death. However, a new systematic review suggests that ICDs may not provide any survival benefit to women with heart failure and left ventricular dysfunction (level 2 [mid-level] evidence). In a subgroup analysis of 934 women from 5 randomized trials with 20-46 months follow-up, there was no significant difference in all-cause mortality comparing ICDs vs. medical therapy (hazard ratio 1.01).  ICDs were associated with lower all-cause mortality in men (hazard ratio 0.78, 95% CI 0.7-0.87) (Arch Intern Med 2009 Sep 14;169(16):1500). Additionally, in a recent cohort of 161,470 patients, periprocedural major adverse event rates were higher for women than for men (2% vs. 1.1%, p < 0.001) (Circulation 2009 Mar 3;119(8):1078).
