Last week 725 articles were evaluated via DynaMed’s Systematic Literature Surveillance and 242 were added to DynaMed content.

 

Based on the editors’ criteria of selecting “articles most likely to change clinical practice,” one article of significant interest was selected for the DynaMed Weekly Update.


Intramuscular Trivalent Inactivated Vaccine may be More Effective than Intranasal Live Attenuated Vaccine for Seasonal Flu in Healthy Adults

 

Intramuscular and intranasal seasonal influenza vaccines were compared to each other and to placebo in a randomized trial with 1,952 healthy adults followed during the 2007-2008 influenza season (N Engl J Med 2009 Sep 24;361(13):1260). The rate of laboratory-confirmed symptomatic influenza was 6.1% overall, with lowest incidence (3.4%) occurring in the group receiving intramuscular inactivated trivalent vaccine (Fluzone) (level 2 [mid-level] evidence). The rate of laboratory-confirmed symptomatic influenza was significantly higher for both live attenuated influenza vaccine (FluMist) (6.9%) and placebo (10.8%). The Number Needed to Treat (NNT) to prevent 1 case of symptomatic seasonal influenza using the inactivated vaccine was 29 compared with live attenuated vaccine and 14 compared with placebo. Arm soreness was the only significant adverse effect associated with the inactivated vaccine. The live attenuated vaccine appeared superior to placebo, with a NNT of 26.
